Structure

The DA Form 5790-R, commonly known as the Record Firing Scorecard for the Scaled Target Alternate Course, serves a crucial role in the evaluation of marksmanship for military personnel. This form is designed for use when standard record fire and known distance ranges are not available, ensuring that soldiers can still qualify their shooting skills effectively. It includes various sections to record firing scores across different positions, such as prone supported, prone unsupported, and kneeling. Each table of the form specifies the number of rounds, time limits, and scoring criteria for engaging targets at varying distances, ranging from 50 to 300 meters. Notably, the form also emphasizes the importance of proper engagement order and time management during qualification exercises. As soldiers progress through the tables, they are scored based on their accuracy, with specific hit thresholds determining qualification levels from expert to unqualified. The form not only facilitates the scoring process but also helps maintain a standardized approach to marksmanship training and evaluation across different units.

Common mistakes

  1. Incorrect ID Code: Many individuals mistakenly enter their Social Security Number (SSN) instead of the required ID code. This can lead to privacy concerns and delays in processing.

  2. Incomplete Scoring: Failing to fill out all the required hit and miss columns for each table can result in an inaccurate qualification score. Each table has specific scoring requirements that must be met.

  3. Missing Evaluator's ID Code: The evaluator's ID code is essential for validation. Omitting this information can cause complications in verifying the scorecard.

  4. Incorrect Date Format: The form requires dates in a specific format (YYYYMMDD). Errors in this format may lead to confusion and affect the record-keeping process.

  5. Failure to Follow Engagement Order: Not adhering to the prescribed order of target engagement can skew results. Engaging targets from left to right and nearest to farthest is crucial for accuracy.

  6. Neglecting to Sign: Many forget to initial or sign the necessary sections of the form. Without proper signatures, the form may be considered incomplete.

  7. Ignoring Time Limits: Each firing table has a strict time limit. Failing to complete the firing within these limits can lead to disqualification from the qualification process.

This scorecard is used to score alternate course record fire qualification when the 25-meter scaled silhouette target (NSN 6920-01-167-1398) is used. The alternate course is used only when standard record fire and known distance ranges are unavailable.

NOTE: If zeroing/grouping exercises are not performed on the day of record fire, six rounds of training/sustainment ammunition will be fired for 25-meter zero confirmation prior to conducting the qualification course.

CONDUCT OF FIRE

Alternate course qualification firers will have one 20-round magazine and two 10-round magazines. To ensure that firers do not forget which targets they engaged and shoot a given target more than the prescribed number of times, firers should adhere to the following guideline: Engage targets on the sheet from left to right and nearest to farthest. Engagement should follow this order: 50m, 100m left, 100m center, 100m right, 150m left, 150m right, 200m left, 200m right, 250m, and 300m.

Though the time between each firing position is not specified, enough time should be allotted to allow the firer to clear his weapon, quickly change firing positions, and reload before beginning the next firing table. The range RSO ensures that enough time is given between each change in firing position to facilitate the timely flow of the record fire qualification table.

(1)Table 1-- Prone Supported Firing Position or (at the unit commander's discretion) Foxhole Supported Firing Position.

The firer is given one 20-round magazine to engage 10 silhouettes on the same target sheet. Table 1 includes 2 rounds for each silhouette. Firing must be completed in 120 seconds. No more than 2 hits are scored for each silhouette.

(2) Table 2 -- Prone Unsupported Firing Position.

The firer is given one 10-round magazine to engage 10 silhouettes on the same target sheet. Table 2 includes 1 round for each silhouette. Firing must be completed in 60 seconds. No more than 1 hit is scored for each silhouette.

(3) Table 3 -- Kneeling Firing Position.

The firer is given one 10-round magazine to engage 10 silhouettes on the target sheet. Table 3 includes 2 rounds for each silhouette positioned at 50 and 100 meters and 1 round for each silhouette positioned at 150 meters. Firing must be completed in 60 seconds. No more than 2 hits are scored for the 50- and 100-meter silhouettes, and 1 hit is scored for each 150-meter silhouette.

SCORING

The same target sheet is used for every 40-round qualification table that a firer completes. One hit is awarded for each round that strikes within or touches some part of the silhouette. A maximum of 40 hits is comprised of 3 hits per target at 200, 250, and 300 meters; 4 hits per target at 150 meters; and 5 hits per target at 50 and 100 meters.

Similar forms

The DA Form 5790-R is similar to the DA Form 5500, which is used for body composition assessment in the Army. Both forms serve as evaluation tools, focusing on performance metrics. While the DA Form 5790-R assesses shooting accuracy and qualification, the DA Form 5500 evaluates physical fitness through measurements of body fat percentage. Each form requires specific data entry, including identification details and performance scores, ensuring that the results are documented consistently and accurately for personnel records.

Understanding the nuances of various forms, including the importance of accurate documentation for transactions, is crucial for maintaining clarity in processes. For instance, the Mobile Home Bill of Sale serves as a vital legal document that ensures the smooth transfer of ownership, much like the essential forms required in military evaluations, guaranteeing precise record-keeping and accountability across different fields.

Another document akin to the DA Form 5790-R is the DA Form 705, which is the Army Physical Fitness Test Scorecard. Like the firing scorecard, the DA Form 705 is used to record performance results. The DA Form 705 evaluates physical fitness through timed events and push-ups, while the DA Form 5790-R focuses on shooting proficiency. Both forms are essential for maintaining readiness and ensuring that soldiers meet the required standards in their respective areas of expertise.

The DA Form 4886, the Family Member Deployment Screening Form, also shares similarities with the DA Form 5790-R. Both documents are utilized to collect important information regarding personnel. The DA Form 4886 gathers data about family members’ health and readiness for deployment, while the DA Form 5790-R collects data related to a soldier’s shooting qualifications. Each form plays a crucial role in ensuring that soldiers and their families are prepared for military service and deployment.

Additionally, the DA Form 2-1, the Personnel Qualification Record, is comparable to the DA Form 5790-R. The DA Form 2-1 provides a comprehensive overview of a soldier’s qualifications, including training and performance evaluations. Similarly, the DA Form 5790-R documents specific qualifications related to marksmanship. Both forms are vital for tracking the readiness and capabilities of military personnel.

The DA Form 2627, which is used for non-judicial punishment, also bears resemblance to the DA Form 5790-R. While the former addresses disciplinary actions, both forms require detailed reporting and documentation. The DA Form 2627 records the nature of the offense and the resulting actions taken, while the DA Form 5790-R logs shooting performance and qualifications. Each form contributes to maintaining standards and accountability within the military.

The DA Form 3508, the Army Commanders' Safety Management System (ACSMS) Risk Management Worksheet, is another document similar to the DA Form 5790-R. Both forms are designed to assess and document performance, albeit in different contexts. The DA Form 3508 focuses on identifying and mitigating risks associated with military operations, while the DA Form 5790-R evaluates shooting qualifications.
