Structure

The Navcompt 3065 form is a crucial document for military personnel seeking to request and authorize leave. This form includes essential information such as the date of the request, the service member's Social Security Number, name, pay grade, and details about their ship or station. It requires the applicant to specify the type of leave being requested, whether it be regular, sick, emergency, or other categories. Additionally, the form captures travel details, including the mode of transportation and the leave address. Applicants must indicate the number of days requested, along with the specific dates and hours for both departure and return. The form also emphasizes the importance of maintaining communication during leave and provides guidelines on how to handle emergencies or changes in travel plans. Furthermore, it incorporates a Privacy Act Statement, ensuring that the information collected is used appropriately and with the service member's consent. Proper completion of the Navcompt 3065 is essential, as it not only serves as an official leave authorization but also outlines the responsibilities and expectations of the service member while on leave.

Common mistakes

  1. Neglecting to Complete All Required Fields: Many individuals forget to fill in essential sections such as their Social Security Number (SSN) or the type of leave they are requesting. Missing information can lead to delays or denials of leave requests.

  2. Incorrect Dates: Entering the wrong dates for leave can create confusion. For instance, if someone mistakenly writes a return date that precedes the departure date, it raises red flags and could result in a rejected application.

  3. Not Following Time Format: When specifying hours and dates, failing to adhere to the required format can cause issues. For example, using a 12-hour clock instead of a 24-hour clock may lead to misunderstandings about when leave begins and ends.

  4. Ignoring Leave Balances: Some applicants overlook checking their leave balances before submitting the form. If a person requests more leave than they have accrued, it could result in financial penalties or the denial of the request.

  5. Not Keeping a Copy: After submitting the Navcompt 3065 form, individuals often forget to keep a copy for their records. This can be problematic if there are questions or disputes about the leave request later on.

  6. Failing to Communicate Changes: If there are changes to the leave address or any other details after submission, not informing the commanding officer can lead to complications, especially during emergencies or recalls.

  7. Submitting Late: Waiting until the last minute to submit the leave request can cause processing delays. It is advisable to submit the form well in advance to ensure that all necessary approvals are obtained in time.

Each form plays a role in ensuring compliance with regulations, whether related to employment or military leave.

The Employee Leave Request form used by many civilian agencies parallels the Navcompt 3065 in its function as a formal request for time away from work.
